TRANSFERRING FROM ANOTHER PRACTICE In order to have your current medical information, please provide us with a copy of your medical records when you transfer to our office. Print out the following form prior to your appointment in order to expedite your initial visit. If you need to cancel or re-schedule an appointment, we ask that you do so at least 24 hours in advance. Our online appointment scheduling service makes this easy to do. It will automatically remind you of your physical exams one week in advance, and regular appointments two days in advance. Fees for "no show" are as follows:
Please let us know if your patient demographics have changed, either with the receptionist when you visit, or by utilizing My Health Portfolio on our website. To speed up your appointment if you have not been to our office in the last three years, please download the form from our website and complete it before you arrive.
