Referrals to Specialists 

Referral requirements to specialists vary with each insurance company. If you need to know the specific requirements for your plan, contact your insurance company.

Referrals issued by us at the time of your visit will be mailed to you within five business days. Ongoing referrals, which need updating for renewal, can be handled by calling our office at 770-513-2072 ext. 205 or by contacting the referral coordinator at My Health Portfolio.

Mammography

Requests for mammography are referred to Gwinnett Hospital System for screening and diagnostic mammograms and other breast imaging procedures. We will provide you with written orders and a number to schedule your appointment with their center.
